Timely reporting of MUIs to Summit DD is crucial to protect those we serve and to help Investigative Agents identify all the relevant causes to an incident for strong prevention planning. Check out these best practice reminders for your reporting.
Promoting timely reporting from our stakeholders is an area of priority for Summit DD’s MUI Team. Here are some statistics on the issue of late reporting from our mandatory reporters:
- In 2024, Failure to Report MUIs are on pace to be the same as last year with about 14.
- In addition to those Failure to Report MUIs, we have had another approx. 64 MUIs reported late by mandatory reporters so far in 2024.
- 28 agencies and six independent providers have been involved with late reports to Summit DD so far this year. Eight agencies have also had between 2-5 late reporting events and two agencies have had 5 or more late reporting events.
All allegations of a suspected MUI need reported to Summit DD by no later than 3pm the first business day after a provider has first learned of the allegation. These written reports can be submitted either by emailing them to MU********@Su******.org or by faxing them to 330-634-8553
These written reports must be submitted on a reporting form with all the required information that is outlined in the MUI Rule. While you do not need to use Summit DD’s reporting form, here is a link to our suggested form.
Some types of suspected MUIs must also be called into Summit DD within the first four hours of a provider’s discovery of the allegation; no matter the time or day they learn of it.
- The number to call during Summit DD’s regular operating hours (typically M-F 8a-4p) is 330-634-8684.
- The Afterhours number to call is 1-877-271-6733 for when the agency is closed.
The only types of MUIs that require this additional four-hour notice are as follows:
- Accidental Death
- Exploitation
- Misappropriation
- Neglect
- Any Peer-to-Peer Acts
- Physical Abuse
- Prohibited Sexual Relations
- Sexual Abuse
- Verbal Abuse
- When the provider has received an inquiry from the media regarding an MUI
4-Hour Reporting Reminders:
- Make sure to leave a Voicemail with your name and a good working phone number if either Hotline is not live answered.
- The other types of MUI not listed above only require the written report be submitted and do not have to be called into Summit DD.
- REMINDER: Even if you have called an MUI in to Summit DD, you are still required to submit a written report by no later than 3pm the first business day after your discovery.
MUI Reporting Best Practices:
- The most important thing to do before reporting is to ensure proper immediate actions have been put in place to protect those we serve. Please remember that reporting the MUI to Summit DD is not an immediate action. For suggestions on strong Immediate Actions, please refer to Summit DD’s Immediate Actions Suggestion Guide
- Include the 5 W’s in your report. Who was involved, What Happened, Where it Happened, When it Happened, & Why it happened if known.
- Do not use generic titles or abbreviations. Use full names of all of those involved. MUI Reports are not public record, so do not be concerned about privacy issues in your descriptions.
