The Summit County MUI Stakeholders Committee met on 3/23/26 to discuss the MUI data and trends from the 2025 calendar year. Learn more about the patterns they identified.
This committee is made up of Summit DD Staff; Agency Provider representatives; Independent Providers; Guardians; & Persons Served by Summit DD.
2025 MUI Data
Overall, Summit DD investigated 1171 MUI categories in 2025. (As a comparison, Summit DD investigated 1479 total MUI categories in 2024.)
- Approximately 43% of those were Category A investigations, with Neglect being the highest instance category with 138 investigations.
- 24% of the total investigations were Category B MUIs, with Significant Injuries being the highest type of Category B incident with 100 total occurrences.
- Lastly, approx. 33% of all investigations from 2025 were Category C MUIs, with Unanticipated Hospitalizations being the most frequent type of Category C MUI at 245 investigations.
The 7/1/25 MUI Rule update had a significant effect on MUIs opened and directly contributed to much of the reduction in categories opened noted in 2025. This is especially true with Unanticipated Hospitalization MUIs; which saw a reduction of 175 investigations in 2025. It should also be noted that almost all Category A MUIs numbers were also down significantly as compared to 2024. This is not directly attributed to the 7/1/25 MUI Rule update however, as that update did not significantly change the definitions of these types of incidents.

Developing Additional Support
Several MUI issues from 2025 were also discussed by the committee. The committee discussed MUIs related to falls and possible causes. Suggestions on ways to support DSPs with preventing falls were provided and will be followed up on by the MUI team over the course of 2026. The committee also discussed some areas teams are having difficulties with new rule implementation. Examples were being able to identify Unanticipated Hospitalization MUIs and better defining what a hostile environment is for Emotional Abuse. Summit DD’s MUI team will take this feedback and work on additional training and/or support documents to assist teams with these areas. This will be developed and released in the near future.
