An application packet will not be ready for review until all supporting documents, including background checks, are submitted. The results of a background check can take up to 45 days to be completed and sent to DODD. Learn more about background check requirements.
BCI Background Check
You will be required to complete a Bureau of Criminal Identification and Investigation (BCI) background check when applying to become a certified provider. *Reason Code: 5123.169
- To find a WebCheck location nearest you click this link to the Ohio Attorney General’s website.
- If you have ever had any criminal convictions – you will need a personal statement written both by you and a court journal with the details of the offense. This includes sealed records.
- You will not receive a copy of your background check, instead it will be sent directly to DODD as part of your provider application.
- Please keep a copy of the receipt from the location where you had the background check completed.
- You must provide the address below to the WebCheck vendor when you have your background check completed. Request that the criminal records check results be sent directly to
Provider Certification, Ohio Department of Developmental Disabilities, 30 East Broad Street, 13th Floor Columbus, OH 43215. (Must be sent directly from the Attorney General’s office to the Ohio Department of Developmental Disabilities Provider Certification.) - Please Note: This process may take-up to 30-45 business days for DODD to receive your background check(s).

FBI Background Check
If you have lived outside of Ohio at any time in the last 5 consecutive years, you will also be required to complete an FBI background check. *Reason Code: 5123.169 or 5126.28
