Most Common Independent Provider Services and Rule Links:
Homemaker/Personal Care (HPC) IO, LV1 Rule
- This is the most commonly provided service in the DD field. Providers will assist people who have identified a need for help to complete daily living tasks, such as hygiene, meal prep, shopping, community living, housekeeping, and other general tasks. Sometimes, the needs may be more involved, such as administering medication or keeping track of finances. This service can be authorized for an ongoing basis, like a weekly schedule, or a more time-limited basis such as a few weekends a year.
Homemaker/Personal Care Transportation (HPC Transportation) IO, LV1, SELF Rule
- This service provides transportation, paid by mile, during HPC services. This mileage can be for errands, appointments, and other outings taken with the person served. This is not for the person’s commute to/from their workplace or day program.
Shared Living (SL) IO only Rule
- This service is specific to a situation where a provider and the person receiving services in the same home. It is to be both the provider and person’s primary residence and is billed as a daily unit. The services look much like HPC, but mimic more of a family setting and supports may be more flexible and service delivery may vary from day to day, depending on the person’s schedule.
Non-Medical Transportation (NMT) IO, LV1, SELF Rule
- This service provides transportation ONLY to a person’s day program, work location, or other regularly scheduled commuter location (volunteer work, internship, etc.) This service can be paid by trip or by mile, based on the person’s authorization. Additionally, this service can be provided by both a non-modified or permanently modified vehicle, at corresponding rates.
Participant Directed Homemaker/Personal Care (PD-HPC) IO, LV1, SELF Rule
- This service includes help with the same types of support as regular HPC. With Participant-Directed Homemaker/Personal Care, a person may decide how much to pay their staff, and they may act as the employer. PD-HPC is available to all waiver enrollees, however, this certification is required to provide services to individuals enrolled in the SELF Waiver program.
Residential Respite IO, LV1, SELF Rule
- This service includes care and support services provided to an individual on a short-term basis because of the absence or need for relief of those persons routinely providing care. Depending on the circumstances of service provision, residential respite is billed at a daily billing unit or at a fifteen-minute billing unit:
Informal Respite LV1 Rule
- This service includes care and support services provided to an individual unable to care for himself or herself, furnished to an individual by a person known to the individual, on a short-term basis because of the absence or need for relief of those persons routinely providing the care.
