MUI Reporting Requirements Reminder

Summit County has seen late or non-reports on the rise since last year. Get tips and helpful resources to curb this negative trend.

Late and Non-Report Trends

In 2024, Summit DD’s MUI department received 90 late MUI reports or non-reports from providers. Through the first half of 2025 there have been 49 late reports received. This puts Summit County on pace for 98 late reports from providers, which is roughly a 9% increase from 2024.

In 2024, written incident reports not being submitted to the MUI department by 3pm the following business day after discovery accounted for 89% of the total. The remaining 11% were late immediate to 4-hour verbal reports. So far in 2025, there has been a slight decrease in late written incident reports (86% of total) and slight increase in late immediate to 4-hour verbal reports (14% of total).

Summit DD’s MUI Intake IAs identified the following common reasons for late or non-reports:

  • A provider not knowing if an incident/allegation met MUI criteria and didn’t report
  • Sending a written report to an erroneous email address or a different county’s MUI department
  • Lastly, for one reason or another, submitting or calling in the report but failing to adhere to the Rule required timeframes.

In 2024 there were 12 Failure to Report MUIs filed. 11 were substantiated. 10 involved DSPs, 2 administrative staff. There were often multiple PPIs per MUI. In 2025, there has been one FTR, that is still open. It involves 2 DSPs.

Reporting Reminders:

  • All suspected MUIs require a written report to be submitted to Summit DD via email or fax by no later than 3pm the first business day after you initially discover an incident. Written reports are required to be in an incident report, and should either be emailed to MUIReports@SummitDD.org or faxed to 330-634-8553.
  • You can find a suggested reporting form for written reports here.
  • In addition to the written reports, the following categories also require reporting via phone call within the first four hours of your initial discover of the incident: Emotional, Physical, & Sexual Abuse; Exploitation; Misappropriation; Any Peer-to-Peer Acts; Prohibited Sexual Relations; Unexplained Death; and Media Inquiries about an MUI.
  • These within 4 hours of initial discovery phone reports must be called into either 330-634-8684 during Summit DD operating hours (typically M-F 8a-4p) or to 1-877-271-6733 when Summit DD is closed, to include Holidays and weekends.
  • Lastly, remember that when in doubt, just report the incident to Summit DD’s MUI team within the required timeframes. You will never be penalized for reporting an incident that does not meet MUI criteria
