You’ll need to gather certain documents before starting your application. Make sure you have the following:
Required documentation for Independent Provider application includes:
- Social Security Card
- Driver’s License (an out-of-state license may delay review)
- Proof of Insurance (only if applying to provide transportation services)
- If your name isn’t on the card, you must show proof you’re listed as a driver on the policy.
- Official Driver’s Abstract (only if applying to provide transportation services)
- Must be requested online from the Ohio BMV.
- Abstracts older than 14 days cannot be accepted. An unofficial abstract cannot be accepted.
- High School Diploma/GED
- Or a letter of support waiving this requirement.
- Birth Certificate
- If born outside the U.S., provide naturalization or citizenship papers showing your birthdate.
- Other Documents (including training certifications or transcripts) may be needed depending on the services you plan to provide.
⚠️ Attention Nurses:
If you hold a current nursing license, you must enroll with the Ohio Department of Medicaid (ODM) before applying to become a DODD provider — no matter which services you plan to offer.
