What to Have Handy Before You Apply for Independent Provider Certification

You’ll need to gather certain documents before starting your application. Make sure you have the following:

Required documentation for Independent Provider application includes: 

  1. Social Security Card
  2. Driver’s License (an out-of-state license may delay review)
  3. Proof of Insurance (only if applying to provide transportation services)
    • If your name isn’t on the card, you must show proof you’re listed as a driver on the policy.
  4. Official Driver’s Abstract (only if applying to provide transportation services)
    • Must be requested online from the Ohio BMV.
    • Abstracts older than 14 days cannot be accepted. An unofficial abstract cannot be accepted.
  5. High School Diploma/GED
    • Or a letter of support waiving this requirement.
  6. Birth Certificate
    • If born outside the U.S., provide naturalization or citizenship papers showing your birthdate.
  7. Other Documents (including training certifications or transcripts) may be needed depending on the services you plan to provide.

⚠️ Attention Nurses:

If you hold a current nursing license, you must enroll with the Ohio Department of Medicaid (ODM) before applying to become a DODD provider — no matter which services you plan to offer.
