Category C Appendix Forms & Proper Reporting

The reporting requirements for all Law Enforcement, Unanticipated Hospitalization, & Unapproved Behavioral Supports (Category C MUIs) changed when the new rule took effect July 2025. Get resources to help with your reporting.

Category C Reporting

For a provider to correctly report these three types of Category C incidents timely, they have to provide a written report as per usual AND the appendix form that corresponds with the category being reported. Part One of these appendix forms needs to be completed and submitted to Summit DD by the reporting provider at the same time as the incident report to be considered reported in compliance with the MUI Rule. Both the incident report and completed Appendix form for Category C incidents needs submitted by no later than 3p the first business day after you discover the incident.

To date almost no Category C incidents reported since July 2025 to Summit DD have included the corresponding Appendix form with Part One completed with the incident reports. This means almost all providers in Summit County that have reported a Category C incident since the new rule took effect, have not reported these types of incidents in compliance with the rule.

DODD Examples

DODD has also developed support for this process. Here is a link to DODD examples of how the forms should be completed and some blank templates that can be used by providers.  There are PDF and Word versions of the templates. Please note that Appendix C is for Law Enforcement; Appendix D is for Unanticipated Hospitalizations; & Appendix E is for Unapproved Behavioral Supports.

Information Expectations

Ultimately it does not matter if you choose to use Summit DD’s template, DODD’s, or if you create your own. However, if you do decide to create your own, you must ensure it contains all the same prompts and questions found in Summit DD’s and DODD’s templates.

Please note

The expectation is that the information provided in the appendix form is more detailed than your summarized incident from the incident report. Also, providing the IA with supplemental documentation to verify what is submitted in the Appendix forms is still required.
