The new MUI rule took effect on 7/1/25. As we all work together to effectively implement these rule changes, Summit DD’s MUI team has created some resources to help providers meet the new rule requirements.

Unanticipated Hospitalization Determination Checklist. With the changes to the Unanticipated Hospitalization rule, it may be more difficult to determine when you have a reportable MUI related to a hospital admission. Appendix forms. Every reported Category C MUI now requires the provider to submit an appendix form with their initial incident report. Providers are required to complete as much of the header as they can and Part One of each form. To help providers with this, Summit DD’s MUI team has created fillable PDFs that can be used for each Category C MUI Appendix form.
